During this process you must also submit to the government an active copy of your workers comp policy.

For any members of your team that are going to be driving around H-2A workers, you must submit a Doctor’s Certificate indicating that the workers are safe to operate a vehicle. The DOT physical which many CDL holders already have will suffice or you can use the WH-515 medical form as well.

The final document you need when applying for FLC Certificates is the IRS Federal EIN proof for the business.
