For any members of your team that are going to be driving around H-2A workers, you must submit a Doctor’s Certificate indicating that the workers are safe to operate a vehicle. The DOT physical which many CDL holders already have will suffice or you can use the WH-515 medical form as well.

The final document you need when applying for FLC Certificates is the IRS Federal EIN proof for the business.
